The Homeless Interagency Solutions Team (HIST) is a team of individuals from a variety of County agencies that come together, anchored by shared values, to identify solutions to homelessness for unhoused adults in our communities. The agencies involved include the Department of Behavioral Health, Department of Human Assistance, Probation Department, Department of Child, Family, and Adult Services, the Public Defender's Office, and the Department of Homeless Services and Housing.

Provide structure and facilitate a service planning process, which includes identifying the strengths and needs of adults experiencing literal homelessness and mobilizing and actionizing resources that augment other agencies' efforts to meet the complex needs of these individuals.​​

To develop individualized person-centered plans for homeless adults that utilize the County's collective capacities and connections to its community partners to identify solutions that address basic needs, foster independence, reduce recidivism, and ultimately place the adult on a pathway to housing.​​

The first and third Thursdays of every month from 1:00 to 3:00pm. The location for each meeting is identified in advance, as it can be subject to conference room availability.​​​

Any agency or service provider working with an adult to identify pathways into shelter and/or permanent housing.​​
